Aging Life Care™ Services is the selection, supervision and coordination of health care services. Horizon utilizes a Masters-level Gerontologist and Registered Nurses.
Custom plans may include:
- Medication management (i.e. med box filling)
- Nurse Wellness checks
- Management of chronic condition resulting in multiple hospitalizations
- Help for families living out of the area or in need of assistance
- Coordination of health care appointments
- Communication among health care professionals and family
- Multiple physician coordination
- Accompanying patient to appointments
- Assistance in determining if current living environment is appropriate and referral assistance if placement is needed
- Budgeting for long-term care needs
- Home safety evaluation and recommendations
